You will receive a secure intake form by email after booking. Complete it at your own pace — it covers your health history, current symptoms, medications, and goals. The more detail you provide, the more productive your consultation will be.

Your first visit is a private conversation — not a rushed clinical exam. We will review your intake, discuss your goals in depth, explain our integrated approach, and outline what a personalized protocol might look like for you. No pressure, no obligation.

If you choose to move forward, we will order a comprehensive diagnostic panel tailored to your goals — hormonal, metabolic, cardiovascular, and neuro-metabolic markers as appropriate. We go well beyond standard bloodwork.

Once your results are in, your clinician will build a precision protocol around your unique biology. Every element — hormonal, psychiatric, and primary care — is coordinated under one plan. You will understand exactly what we are doing and why.

Your protocol evolves as you do. We monitor your biomarkers, adjust as needed, and remain accessible between visits. This is concierge medicine — you have a direct relationship with your clinician, not a rotating roster of providers.
Intake Preparation
Gathering this information before your visit helps us make the most of your time together.
A summary of past diagnoses, surgeries, hospitalizations, and significant health events. Include family history of cardiovascular disease, cancer, diabetes, or mental health conditions.
A complete list of all prescription medications, over-the-counter drugs, vitamins, and supplements — including dosages and how long you have been taking them.
Any bloodwork, hormone panels, or diagnostic results from the past 12 months. Even if results were "normal," we may interpret them differently through an integrative lens.
Think about what you most want to change or improve — energy, mood, body composition, cognitive clarity, sleep, libido, longevity. The more specific, the better.
Note any symptoms you experience regularly, even if they seem unrelated or minor. Fatigue, brain fog, weight changes, mood shifts, and sleep disruption are all relevant.
